        1. 2x2 or 1x1 ID Picture

        2. Valid Photo Bearing ID

        3. Certification from the Head of Office / Latest appointment duly certified by HRMO

        4. Payroll or Pay slip (3 months) / Any Proof of Income / Certificate of Remuneration / Bank Statement / ITR

        5. Agreement Payroll Deduction / ATM photocopy attached with affidavit of voluntary surrender for ATM

        6. Co-maker Statement
