• 1 valid photo bearing ID

• 2x2 or 1x1 ID Picture (4pcs)

• Any Proof of Income / 3 months Latest Payroll or Payslip/ ITR/ Certificate of Remuneration.

• Certification from the Head of Office or Latest Appointment Duly Certified by HRMO.

• 1 qualified co-maker (with Valid Photo Bearing ID and Proof of income)

• Duly signed Agreement Payroll Deductions (Head of Office and Paymaster)/ ATM photocopy with attached affidavit of voluntary surrender for ATM.
